STORY & MEDIA RELEASE

RELEASE FOR SHARING STORY 

I, the undersigned, do hereby consent and agree that CDAIDE, or its agents, have the right to share details of my personal story for the purposes of raising awareness of CDAIDE’s work in the community, helping others to understand the challenges faced by local restaurant and hospitality workers, and raising funds to support restaurant and hospitality workers through CDAIDE. I further consent that my name, identity, work place, and personal challenges may be revealed therein or by descriptive text or commentary.

I do hereby release to CDAIDE or its agents all rights to share my story in print and electronic form both publicly and privately. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

I understand that there will be no financial or other remuneration for sharing my story. I also understand that CDAIDE is not responsible for any expense or liability incurred as a result of my participation, including medical expenses due to any sickness or injury incurred as a result. 

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement. I also understand that I may revoke this agreement at any time by contacting CDAIDE except in instances where action has already been taken in accordance with this agreement.

RELEASE FOR MEDIA RECORDING

I, the undersigned, do hereby consent and agree that CDAIDE, or its agents, have the right to take photographs, videotape, audio or digital recordings of me and to use these in any and all media, now or hereafter known, and exclusively for the purpose of community education and awareness, and fundraising or other public events. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

I do hereby release to CDAIDE or its agents all rights to exhibit this work in print and electronic form both publicly and privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

 
I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.

I also understand that CDAIDE is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result.
